CUIDADO FARMACÊUTICO NO TRANSPLANTE DE CÉLULAS-TRONCO HEMATOPOIÉTICAS NO CEARÁ

Abstract
Objectives: To report the experience, evolution and consolidation of pharmaceutical services provided to patients undergoing hematopoietic stem cell transplantation (HSCT) at a university hospital complex in Fortaleza (CE), Brazil. Materials and methods: This report was constructed based on the experience lived by pharmacists of the bone marrow transplant service, as well as by resident pharmacists, during the period from January/2016 to December/2017. The services offered were: health education; dispensation; conciliation and therapeutic monitoring of medications; review of pharmacotherapy; pharmacotherapeutic follow-up and health condition management. Results: It was found that 17 patients hospitalized for HSCT were followed up, from admission conciliation to discharge guidance. At the outpatient clinic, 34 patients were followed up, with 301 pharmaceutical consultations being carried out with recommendations, the main ones being: health care (28.3%) and guidance on medication acquisition (25.5%). Conclusion: Despite the limitations found, this work contributed to the development of the discussion regarding specialized care for HSCT patients and to demonstrate the importance of the pharmacist participating in this care with the multidisciplinary team
